COVID-19 Out of Province Dose Documentation

Complete the form below to notify Ottawa Public Health (OPH) of your (or your child’s) out of province dose administration if you would like them to appear in the provincial COVID-19 record system.

OPH will only process COVID-19 vaccine records submitted through this form for:

  • clients under 5: any doses received out of province
  • clients 5 and older: doses received out of province in the last 6 months.

If your proof of vaccination is handwritten, please provide further documentation, such as:

  • Clinic contact information (Please note that we may contact the clinic to verify your record)
  • Proof of travel
