COVID-19 Dose Documentation - Chosen Name(s) or Legal Name Change

Ontarians who are in the process of changing their legal (current) name to their chosen name or who have completed a legal name change can request to have their name changedon their COVID-19 proof of vaccination. Please complete the form below to notify Ottawa Public Health of your name change and upload documentation to support this request.

Legal name change documentation must include valid government issued identification such as such as a health card, birth certificate, immigration document, passport etc.

Chosen name documentation can include those listed on this Ministry of Health resource “Updating your Ontario COVID-19 Vaccine Certificate to your Chosen Name”
