Complete this form for one of the following reasons:

• If you do not know your child’s Ontario Immunization ID (OIID), or you have never received an OIID via a surveillance letter in the past where it would be listed.
• If your child has recently had their 16th birthday and the auto-reset for your ICON access has taken place. You will now need your child’s permission to access their Immunization record and to reset the OIID to continue accessing their vaccination record through the ICON portal.

You should only submit this form to Ottawa Public Health if you meet one of the following criteria:

• You are the legal guardian or parent of a child aged 15 years or younger.
• You are the legal guardian or parent of a child aged 16 years or older, and have obtained their permission to access their vaccination record via ICON.
• You are the client and are aged 16 years or older and want to access your vaccination record via ICON.
