Ontario Immunization ID Request/Reset Form - Child

Complete this form for one of the following reasons:

• If you do not know your child’s Ontario Immunization ID (OIID), or you have never received an OIID via a surveillance letter in the past where it would be listed.
• If your child has recently had their 16th birthday and the auto-reset for your ICON access has taken place. You will now need your child’s permission to access their Immunization record and to reset the OIID to continue accessing their vaccination record through the ICON portal.

You should only submit this form to Ottawa Public Health if you meet one of the following criteria:

• You are the parent or legal guardian of a child under 16 years of age who lives or goes to school in Ottawa. OR
• You are the client requesting your own record, are 16 or 17 years of age and live or go to school in Ottawa.
• The information you provide in this form matches the information in the provincial database.
• Records exist in the provincial database.

**If you had previously met the above criteria but have since moved out of province, please continue, and enter the last address you had in Ottawa.
