Complete this form if you do not know your Ontario Immunization ID (OIID), and/or would like to reset the PIN number.

You should only submit this form to Ottawa Public Health if you meet the following criteria:

• You attended grade school or high school in Ottawa.
• The information that you provided in this form matches the information in the provincial database.
• Records for you exist in the provincial database.
• You are 18-27 years of age.

If you do not have an immunization record with us, or the information you provided cannot be matched to the provincial database, you will receive an e-mail of what you can do next.
