Complete this form once your clinic has been onboarded to submit your list of users for onboarding into COVax.

Please submit a list of all names for COVaxON registration along with their User Profile (key role) as in the template below; one row per person.

Once the agreement is signed, Ottawa Public Health will arrange for COVaxON registration; training will be provided by OntarioMD.

The 'COVaxON Site Staff' and 'COVaxON Vaccinator' profiles will meet your needs. The definitions of these two roles are provided below to allow you to better select the profile needed for each of your for COVaxON users:

  • COVax Site Staff: Provides the user with (1) create/read/edit access to Client screen and data via Client Search across all sites; (2) create/read/edit access to Check-in and Check-out screens, and data across all sites; (3) read access to Vaccine Administration screens and data across all sites; (4) read access to Inventory screens and data across all sites; (5) view and filter the Dashboard, including drill down to PHI; (6) view and filter all reports (except Vaccine Inventory Report), including drill down to PHI, where available
  • COVax Vaccinator: Provides the user with (1) create/read/edit access to Client screen and data via Client Search across all sites; (2) create/read/edit access to Check-in and Check-out screens and data across all sites: (3) create/read/edit access to Vaccine Administration screens, and data across all sites; (4) read access to Inventory screens, and data across all sites; (5) view and filter the Dashboard, including drill down to PHI
