Small Drinking Water System (SDWS) Closure Form

This form can be used if you want to notify the Health Unit the date your small drinking water system closure took place. Only use this form if the closure is for more than 60 days. When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself.

If you are providing a closure notification for more than one small drinking water system, this form should be completed for each small drinking water system.

This information is collected under the authority of the Health Protection and Promotion Act, R.S.O. 1990, c.H.7, Ontario Regulation 319/08 for the purpose of inspecting and making recommendations regarding the operation of small drinking water systems in the City of Ottawa.  Any questions should be directed to the Program Manager, Public Health Inspection Branch.
