Should you be reporting an outbreak or a positive influenza case on Evenings, weekends or holidays, call 3-1-1 and ask to speak with the Public Health Inspector on-call.

The information collected in this form will not be used to report publicly about a possible outbreak in your setting. Only confirmed outbreaks are publicly reported.

If you note an increase in either respiratory or enteric illness from baseline and would like assistance to determine if an outbreak is occurring, please complete form to the best of your ability.

Congregate living settings must report suspected outbreaks of illness to OPH. The following should be reported in this section:

  - Any client or staff tests positive for COVID-19 via PCR or rapid antigen test (RAT)
  - There is a suspected outbreak of respiratory illness in the setting, i.e.:
    * Two or more cases of respiratory illness within a 48-hour period
    * Note: this includes individuals with respiratory illness who have tested negative for COVID-19
  - There is a suspected outbreak of gastrointestinal illness (for example, vomiting/diarrhea) in the setting, i.e.:
    * Two or more cases of gastrointestinal illness within a 48-hour period
