General Contact Upload Form

Thank you for agreeing to complete the contact list. The process of finding people at risk of acquiring the COVID-19 virus is called contact tracing. Contact tracing helps people get tested early and reduces the opportunity for the virus to spread.

Please complete the list to the best of your abilities. Kindly return the form to OPH within 24 hours by uploading the list here. It is important to complete the top section of the list that includes:

For the contact listed, at a minimum include: 

  • the person’s first name
  • a current phone number and/or an email address
  • the relationship (e.g., employee, attendee, coach)
  • last day for exposure: the last day each person was in contact with the person who received a positive COVID-19 test

If possible, please also include:

  • the person’s email
  • the person’s date of birth, gender, address
  • The reference number from the email
  • The name of the setting (i.e., name of the sports team/event/workplace)
  • The address of the setting (i.e., address of sports team/event/workplace)
  • Your full name and contact information including your phone number and email

Information collected on this encrypted form will only be used for the purpose of determining your health care needs.
